29. Miscellaneous Requirements. In addition to the references and information set out in article 353 of the Code of Civil Procedure (chapter C-25.01), those required by section 9 of this regulation, if applicable, shall be indicated in the notice of appeal and the application for leave to appeal.
The second and third paragraphs of article 358 of the Code of Civil Procedure and section 38 of this regulation shall be reproduced at the end of the notice of appeal.
